The MyRadio™ Music For Life Sweepstakes™

1. Beginning 12:01 PM (EST) 10/15/99 through 11:59 PM (EST) 7/01/00, you may enter the NextAudio "Music for Life" Sweepstakes by visiting our web site at www.nextaudio.com or www.myradio.com. Once at the URL, follow the on-screen instructions for entering the sweepstakes. You may also enter by mail by HAND-PRINTING your name, complete address (including zip or postal code), age, daytime phone number, e-mail address, and musical preference on a 3" X 5" piece of paper and mailing it to: The NextAudio "Music for Life" Sweepstakes, P.O. Box 8556, Prospect Heights, IL 60070. Limit one entry per outer mailing envelope. Mailed entries must be postmarked by 7/01/00 and received by 7/10/00. No mechanically reproduced or photocopied entries allowed. Web-site entries must be received by 11:59 pm (EST) 7/01/00. Please limit web site entries to one per day.
   
   
2. Winners will be selected in a random drawing conducted on or about at 7/11/00 by Promotion Activators, Inc., an independent judging organization whose decisions will be final in all matters relating to this sweepstakes.
   
   
3. One (1) Grand Prize: 1,000 CDs. Approximate retail value: $15,000.00.
   
   
4. Sweepstakes open to legal residents of the U.S. of any age, except employees of Fusion Ventures, its portfolio companies, Netgift Registry, NextAudio, their affiliates, subsidiaries, advertising and promotion agencies, and the immediate families of each. Void where prohibited. All federal, state, and local laws and regulations apply. Entries become the sole property of the Sponsor and will not be returned. Grand Prize winner will be notified by Certified mail and must sign and return an affidavit of eligibility, liability/publicity release (where legal), and prize acceptance form for delivery to sweepstakes headquarters within 21 days of notification (first attempted delivery. In the event of noncompliance with these requirements, winner may be disqualified and an alternate selected. A prize won by a minor will be awarded to a parent or legal guardian who must execute and return an affidavit of eligibility and liability/publicity release on winner's behalf within 21 days or prize notification.
   
   
5. Sponsor is not responsible for entries not received due to lost, failed, delayed or interrupted connections or miscommunications, to other electronic malfunctions or errors. Sponsor is not responsible for lost, late, mutilated, illegible, incomplete, postage due or misdirected mail or entries. Sponsor not responsible for incorrect or inaccurate entry information, whether caused by web site visitors or by any of the equipment or programming associated with or utilized in the sweepstakes or by any human error which may occur in the processing of the entries in this sweepstakes. Sponsor reserves the right, in its sole discretion, to cancel or suspend this sweepstakes should a virus, bugs or other causes beyond the control of the Sponsor corrupt the administration, security or proper operation of the sweepstakes.
   
   
6. All federal, state, and local taxes on prize, if any, are the sole responsibility of the winner. Transfer of prize is not permitted. No prize substitutions except by sponsors whom may substitute a prize of equal or greater value if advertised prize becomes unavailable. Prize will be awarded provided a sufficient number of eligible entries are received. Odds of winning depend on the number of eligible entries received. Acceptance of prize constitutes permission to use winner's name and/or likeness for advertising/trade purposes without additional compensation, except where prohibited.
